Ignore replacing ssh host that fixes limited network

ssh.github.com is a hack for people on limited networks to be able to use SSH through HTTPS port. We should ignore replacing it when constructing URLs.

As part of the fix, refactor into SSHConfig and SSHConfigReader to make code testable. It also fixes a bug on `hostReStr` that doesn’t match the “Hostname” string in ssh config.

Add example for issue number

Technically, anything in the third argument is passes as a string, so you can look at any file if you know the hash (for instance, `blob/HASH/app/index.js`. However, having the issue number passed seems to me to be something really useful to my workflow, so I was hoping to find something in the documentation about it. 

Another possibility would be to enable a fourth argument for issue number. I'm happy to do a PR for this if something things it might be of more worth than just this edit.

Fix wrong URL of current user for GHE

- "git protocol for read and ssh for write" had introduced
  additional expectations of behavior.

GHE is using /api/v3 as the path prefix for all API URLs. There’s one miss case when getting current user.

Fix broken colorized verbose output
