One point is to have less places where we actually need tty pointer.
The other is that low_latency is bound to buffer processing and
buffers are now in tty_port. So it makes sense to move low_latency to
tty_port too.

Add SCLP PCI configure/deconfigure and implement a PCI hotplug
controller (s390_pci_hpc). The hotplug controller creates a slot
for every PCI function in stand-by or configured state. The PCI
functions are named after the PCI function ID (fid). By writing to
the power attribute in /sys/bus/pci/slots/<fid>/power the PCI function
is moved to stand-by or configured state. If moved to the configured
state the device is automatically scanned by the s390 PCI layer.

After commit "TTY: move tty buffers to tty_port", the tty buffers are
not freed in some drivers. This is because tty_port_destructor is not
called whenever a tty_port is freed. This was an assumption I counted
with but was unfortunately untrue. So fix the drivers to fulfil this
assumption.

To be sure, the TTY buffers (and later some stuff) are gone along with
the tty_port, we have to call tty_port_destroy at tear-down places.
This is mostly where the structure containing a tty_port is freed.
This patch does exactly that -- put tty_port_destroy at those places.

The 3215 console always has the RAW3215_FIXED flag set, which causes
raw3215_shutdown() not to wait for outstanding I/O requests if an attached
tty gets closed.
The flag however can be simply removed, so we can guarantee that all requests
belonging to the tty have been processed when the tty is closed.

However the tasklet that belongs to the 3215 device may be scheduled even if
there is no tty attached anymore, since we have a race between console and tty
processing.
Thefore unconditional tty_wakekup() in raw3215_wakeup() can cause the following
NULL pointer dereference:

The easiest solution is simply to check if tty is NULL in the tasklet.
If it is NULL nothing is to do (no tty attached), otherwise tty_wakeup()
can be called, since we hold a reference to the tty.
This is not nice... but it is a small patch and it works.

* we give the TTY layer a tty_port
* we do not find the info structure every time open is called on that
  tty

In this case ->install is the only thing we want to do. We do not need
->open at all. See the tty->count > 1 check.

And since we take a reference in ->install, we need also ->cleanup to
drop the reference to a view.

Final note, see that we leave raw3270_find_view in place. It is
because views are removed even from module_exit.

raw3215[line] is set in probe, but not unset in remove. This will lead
to random crashes if the device is removed and the corresponding tty
opened later. open would dereference freed memory.

So set raw3215[line] to NULL in remove to fix that.

If the last block of the HSA is read, EVSTATE_ALL_STORED is returned
by SCLP. Because of a missing break in the switch statement two trace
entries are written in this case: "all stored" and "part stored".

This patch adds the missing break and also adds a "fall through"
comment to improve the readability.

Remove the file name from the comment at top of many files. In most
cases the file name was wrong anyway, so it's rather pointless.

Also unify the IBM copyright statement. We did have a lot of sightly
different statements and wanted to change them one after another
whenever a file gets touched. However that never happened. Instead
people start to take the old/"wrong" statements to use as a template
for new files.
So unify all of them in one go.

Currently qemu/kvm on s390 uses a guest mapping that does not
allow the guest backing page table to be write-protected to
support older systems. On those older systems a host write
protection fault will be delivered to the guest.

Newer systems allow to write-protect the guest backing memory
and let the fault be delivered to the host, thus allowing COW.

Use a capability bit to tell qemu if that is possible.

Add missing storage key initialization when memory comes online.
Didn't hurt until now, since the kernel always uses access key 0.

However for debugging purposes we sometimes use a different key
and it might be good if everything is properly initialized...

raw3215_next_io. Other than that, we just use the tty with raised

There are two copies of allocations of device information. One of them
is totally broken. See:
raw->cdev = cdev;
raw->inbuf = (char *) raw + sizeof(struct raw3215_info);
memset(raw, 0, sizeof(struct raw3215_info));

It suggests that this path was never executed. The code uses both
raw->cdev and raw->inbuf all over. And it is NULL due to the memset
here, so it would panic immediately. I believe nobody used that driver
without being a system console.

Either way, let us fix it by moving the allocations (and
initializations) to a single place. This will save us some double
initializations later too.

And while at it, initialize the timer properly -- once, at the
allocation.
