Add platform-specific callback functions for interrupts.  This is
needed to do a single read-clear of the CAUSE register and then call
out to platform specific functions for DB threshold interrupts and DB
drop interrupts.

Add t4_mem_win_read_len() - mem-window reads for arbitrary lengths.
This is used to read the CIDX/PIDX values from EC contexts during DB
drop recovery.

Add t4_fwaddrspace_write() - sends addrspace write cmds to the fw.
Needed to flush the sge eq context cache.

The driver keeps a bitmap of the netdevs it registered so it knows what to
unregister later.  Remove that and look at reg_state instead.

Remove a field the driver uses to keep track of the name of the first
netdev it manages to register.  Do this by changing the registration
loop to stop the first time it fails so the first registered device is
trivial to tell.

Currently MSI-X names for netdevs with long names are truncated in
/proc/interrupts due to insufficient space.  Use IFNAMSIZ to size the
needed space.

Currently the driver assumes that queue IDs start at 0 but that's true
only for function 0.  To support operation on other functions get the
start of the queue ranges from FW and offset accordingly.

Implement the get_rxnfc, get_rxfh_indir, and set_rxfh_indir ethtool

Handle the larger flash memories on newer boards:

- get the size and number of sectors by probing the flash
- writes and erases can take longer, adjust the timeouts for these operations
- the FW image can be at different locations depending on flash size,
  find its location dynamically as well.

The PF driver needs to remain alert while its ports are down to service
requests or errors from virtual functions or FW so keep interrupts and
queues available when the ports are brought down.  The change makes
open_device_map unnecessary so remove it.
