begin restoring the test_api contract interface, pull results out of the apply_context into chain controller

Enable building and startup of net_plugin. Some changes were made to accomidate all the changes needed for block summaries but ultimately this commit has disabled block sumarries for the moment.

Added test for basic wasm contract loading/unloading.  Fixed bugs in the intrinsic code and made it impossible to complile with the same class of bugs in the future.  Did some basic style/renaming in the contracts directory and in the enclosed tutorial/doxygen files

STAT-153(GH-682): Remove signed_transaction from transaction_object so it can be stored in shared memory

- rename eosc to eosioc
- rename eos-walletd to eosio-walletd
- fix wallet plugin
- fix producer plugin
- eosiod will now produce blocks every 500ms
- rename eos_chain to eosio_chain
- move include/eos to include/eosio for various libraries

- adding block merkle root incremental calculation and validation

switch to using unique_ptrs for intrinsic functions, the internal class for that library has copy and move constructors but if they are used then the classes will segfault on destruction due to the use of a singleton mapping that breaks semantics necessary for copies and moves to work
