Make currency contract tables primary keys same as eosio.token.

Removed mindelay action since the `delay_sec` field makes it unnecessary.

Additional unit tests to test the new semantics of delay_sec are needed.

Also, apply_block and push_transaction now respect the skip_authority_check flag.

compress context_free_data in packed_transaction and change bandwidth usage values in transaction header into upper limits

remove testers estimation of cpu usage which was horribly broken by 18009d1e but not detected due to a math error put into the chain_controller.  Also fixed tests whose assumptions were now wrong after the removal of said estimation.  Finally, fixed the dice contract which was expecting empty tables to exist. EOSIO/eos#2042

multiple wasm_cache_impl structures was causing pre-mature garbage colleciton of all of WAVMs objects.  There is now a RAII guard whose lifetime extends to cover any/all alive wasm_cache_impl structures to make sure we do not pull the GCObjects out from underneath a cache.  This means that no GC will run until there is a point in time where no wasm_cache_impl exists

Change push_nonce, now called push_dummy, test code to use a require auth action since nonce is context free.

Make a way to configure the wasm runtime used during chain_test. This small change is probably "good enough" since the CI checks will use ctest which has now been instructed to run chain_test twice -- once for each runtime. Trying to make chain_test run all its tests twice automatically was tricky and complex
