Add debounce for schema changes to fix slowness when launching with the settings editor persisted open

before the initial render, so we will only render once
when we get the search results.
Also, have to be sure to retrigger search whenever there is a
schema change so extension results show up

TS 3.7 changes the many dom style typings to just be `string` instead of `string | null`. Use empty string in place of null for these cases

Remove focus outline on settings editor. It's not tabbable so it doesn't serve any purpose and is annoying to see every time I click on the editor background. Just need to keep .editor-instance from getting focus on click.

Avoid updateTreeScrollSync when TOC doesn't have the same elements as the settings tree.
And ensure that group counts are always up to date when refreshing the TOC
