Update docs to mention @HeaderMap

Fix typo in README of retrofit-mock.

* Don't catch fatal errors.

* Fix more errors slipping in.

* Add permalink.

* Call errors fatal instead of unrecoverable.

* Rename caught exception variable.

* Don't catch AssertionErrors in tests.
