Using %pV reduces the number of printk calls and
eliminates any possible message interleaving from
other printk calls.

Also improve ath_opmode_to_string usage by having it return UNKNOWN
rather than NULL in the event of failure to map the opmode value to a
representative string.

ath9k uses this for now, ath9k_htc is expected to re-use this
as well. We lave ath5k as is, but it certainly can also be
converted later.

The ath9k module parameter and debugfs entry is kept.

Initial dump of the otus USB wireless network driver.
It builds properly, but a lot of work needs to be done cleaning
it up before it can be merged into the wireless driver tree.
