x86_64: introduce CalIOC2 support
CalIOC2 is a PCI-e implementation of the Calgary logic. Most of the programming details are the same, but some differ, e.g., TCE cache flush. This patch introduces CalIOC2 support - detection and various support routines. It's not expected to work yet (but will with follow-on patches).
