Make the needed changes for the new scheme especially around PHY
initialization, and link detection.
- The get_emac_base function was changed to enable setting different MAC
access setting for each PHY based of external configuration.
- As a part of the dual-media preparation, the save_spirom_version was
modified to accept a PHY specific version address to be saved in the
shmem. This will enable to save more than single spirom version in

different locations.
- The test_link function was changed also as a preparation for the
dual-media upcoming changes
- Duplicate initialization logic was removed from the
link_setting_status and link_initialize functions

Move the code into PHY oriented functions, and for that a new structure
is defines for each PHY which contain PHY properties and its own
functions. This also enables to encapsulate all PHY specific operations
into the PHY functions. During initialization, the PHYs will be probed
by the "bnx2x_phy_probe" function to detect which PHYs exist on-board,
and configure them accordingly. Note that the ext_phy_reset
implementation was incorporated in the ext_phy_init since it is actually
part of the PHY initialization procedure.

Start building the infrastructure for dual media by adding new component

- Advertise 1G KX4 in CL73 when 1G speed capability is enabled
- Add flow-control negotiation over CL73
- External loopback test on Serdes should be done in FORCE mode, since in
CL73 it is unable to link up with the same core using AUTONEG
- Fix bnx2x_set_led function to support CL73 link leds

There are 3 operations that the driver needs to support to allow applications to
access the PHY FW (on top of the MDC/MDIO access). Since those are essentially
nvram access commands, adding them to the ethtool -E interface.

Adding support for BCM8727 - a dual port SFP+ PHY. That includes verification of
the optic module vendor and part number - the list of approved modules resides
on the nvram and the module is verified by the FW. Since not all users would
like to use this verification feature, it can be disabled. The default behavior
is to issue a warning if the module is not approved, but still allow using it -
but it is also possible to disable the link if the module is not approved.

Also adding the ability to recognize the optic module and disable it if it is
not authorized for safety reasons - since this feature might upset some users
which are willing to take the risk, it is optional and can be disabled by
setting an nvram bit (or a trivial driver patch to set this bit).
This dual port PHY requires special handling if the ports are swapped.

These are now defined in linux/mii.h and the bnx2x driver
defines different values which are shared with hardware
data structures.

So add a "BNX2X_" prefix to these macro names.

8073 PHY changes
The initial support we had for this PHY needs some serious changing. The
major change is that this PHY should be initialized only when the first
function is loaded and not for each function. The official SPI-ROM of
this PHY was released and it requires some changes in the initialization
code as well

Pause settings
- 1G pause was not working due to missing write to the emac block
  (TX_MODE_FLOW_EN)
- The flow control should use the negotiated result (after autoneg) so
  we should save both the requested autoneg and the result
- The HW credits with flow control at 1G speed were not optimized and
  caused low throughput
- It is recommended to turn off flow control if the MTU is bigger than
  5000B due to internal buffers size
