The g_std and s_std operations are video-related, move them to the video

VIDIOC_DBG_G_CHIP_IDENT has been replaced by VIDIOC_DBG_G_CHIP_INFO. Remove
g_chip_ident support from bridge drivers since it is no longer needed.
This patch takes care of all the trivial cases.

- open/close/read and poll need to take the core lock as well.
- when the tuner goes to sleep we should set std_set_in_tuner_core
  to 0 since the tuner loses the firmware at that time.
- initialize the tuner if std_set_in_tuner_core == 0 whenever:
  1) g/s_tuner, s_std or s_frequency is called
  2) read or poll is called
  3) streamon is called

- V4L2_CAP_AUDIO was set, but enumaudio was not implemented.
- audioset was never filled by enum_input
- ctrl_ainput was never updated when switching the video input
- g_audio was broken due to faulty logic: g_audio should set the
  index, it doesn't receive it from the user.

- define an initial frequency
- return an error if g_frequency is called for an invalid tuner index
- get the clamped frequency value after setting it: i.e. the tuner driver
  may clamp the given frequency to a valid frequency range and ctrl_freq
  should get that actual clamped frequency.
- remove obsolete tuner type checks (done by the core).

We weren't checking whether the resource was in use before calling
res_free(), so applications which called STREAMOFF on a v4l2 device that

The original implementation of the analog support would use an
i2c_gate_ctrl function when using the digital side of the au8522, but on
the analog side we would always just force the gate open and leave it
open all the time.

This can have adverse effects on the xc5000 given the tuner is receiving
all the spurious i2c traffic (a problem which can be exaggerated due to
bugs in the au0828 i2c hardware implementation).

Rework the existing hack to only open/close the gate when actually
talking to the tuner.

This logic might need to be reworked a bit if anybody ever tries to add
support for a board that has the au0828/au8522 but doesn't have digital
support implemented (because the i2c_gate_ctrl callback is being set in
the DVB attach).  However given how few different models are in
circulation, this can be deferred until such a situation arises (if
ever).

If the user performs a tuning attempt without explicitly calling the
s_std ioctl(), a value of zero is sent from tuner-core to xc5000.  This
causes the xc5000 driver to leave the standard unchanged.  The problem
was masked by the fact that the xc5000 driver defaulted to NTSC, but if
you happened to perform an ATSC/ClearQAM tuning attempt and then do an
analog tune, the net effect is an analog tune with the standard still
set to DTV6.

Keep track of whether the standard has ever been sent to tuner-core.  We
don't make an s_std subdev call explicitly during probe because that
will cause a firmware load (which is very time consuming on the 950q).
With the logic in this patch, the s_std call will occur automatically on
the s_freq call if it hasn't already been set.

In some cases users would see the xc5000_attach() call failing for the
digital side of the tuner on initialization.  This is because of udev
running v4l-id while the digital side of the board is still coming up.

This is the exact same race condition which was present in em28xx (not
surprising since I copied all the locking logic from that driver when I
added analog support).  Reproduce Mauro's fix from the em28xx driver in
au0828.

At the point of the call to video_register_device, both dev->vbi_dev and
dev->vdev have been allocated, and so should be freed on failure.  The
error-handling code is moved to the end of the function, to avoid code

All the modified drivers didn't have any version increment since
Jan, 1 2011. Several of them didn't have any version increment
for a long time, even having new features and important bug fixes
happening.

As we're now filling the QUERYCAP version with the current Kernel
Release, we don't need to maintain a per-driver version control
anymore. So, let's just use the default.

In order to preserve the Kernel module version history, a
KERNEL_VERSION() macro were added to all modified drivers, and
the extraver number were incremented.

I opted to preserve the per-driver version control to a few
pwc, pvrusb2, s2255, s5p-fimc and sh_vou.

A few drivers are still using the legacy way to handle ioctl's.
So, we can't do such change on them, otherwise, they'll break.
Those are: uvc, et61x251 and sn9c102.

The rationale is that the per-driver version control seems to be
actively maintained on those.

Yet, I think that the better for them would be to just use the
default version numbering, instead of doing that by themselves.

While here, removed a few uneeded include linux/version.h

au0828: fix VBI handling when in V4L2 streaming mode

It turns up V4L2 streaming mode (a.k.a mmap) was broken for VBI streaming.
This was causing libzvbi to fall back to V4L1 capture mode, and is a blatent
violation of the V4L2 specification.

Make the implementation work properly in this mode.

We need the au0828 to continue delivering frames even when the device is not
delivering video, or else applications such as tvtime will block indefinitely.

Unfortunately, the au8522 doesn't have any sort of free-running mode or "blue
screen on no video" like some other decoders.
