__get_free_pages() doesn't return HI memory, so the memory is always mapped.
kmap_atomic() is not necessary here. This patch removes the kmap_atomic()
calls and related code for locking and page manipulation.

The ring buffer is only used to pass meta data for outbound packets. The
actual payload is accessed by DMA from the host. So the stop/wake queue
mechanism based on counting and comparing number of pages sent v.s. number
of pages in the ring buffer is wrong. Also, there is a race condition in
the stop/wake queue calls, which can stop xmit queue forever.

The new stop/wake queue mechanism is based on the actual bytes used by
outbound packets in the ring buffer. The check for number of outstanding
sends after stop queue prevents the race condition that can cause wake
queue happening earlier than stop queue.

all references to skb frags size.

Define accessors : skb_frag_size() to fetch frag size, and

Now, eliminate the usage of ext field in struct  hv_device for netvsc driver.
We do this by registering pointer to struct netvsc_device as the driver
specific data and eliminating the current usage of driver specific data
to save and retrieve the pointer to struct net_device.
Additionally, all access to the driver specific data is through
the vmbus wrapper functions. As part of this cleanup, we also get rid
of some unnecessary debug print statements.

Because the number of available buffer slots doesn't decrease for failed
sends, we should not call netvsc_xmit_completion(), which increase the
count of available slots. In this failed case, just free the memory is
enough.

netif_notify_peers() caused a kernel warning in netvsc_linkstatus_callback(),
because netvsc_linkstatus_callback() is within IRQ context. So we move
the first call to netif_notify_peers() into queued work as well, but with
zero delay.

In addition to "staging-next", this should also be back-ported to stable
kernels 2.6.32 and later.

Individual drivers should never be touching the 'struct device' field,
so if that is a requirement to pass to the vmbus core, you know
something is wrong.

This patch fixes that all up, and resolves the problem where the module
reference counting was not happening properly for the individual drivers
as well.  Overall, it reduces the lines of code the individual drivers
have to have, which tells you that this is the correct thing to do.

Also, somehow the _GPL marking for the functions got removed on an older
patch.  As the name of the function was changing, properly change the
_GPL marking as well at the same time.

The transmit slots were manipulated without proper locking. Fix this bug by
making the variable tracking the transmit slots atomic.

This patch should be ported to prior stable kernels 2.6.32 and later.
