We have flattened the BFA hierarchy and also reduced the number of
source and header files we used to have earlier.

Add new bfa functionality to support dynamic queue selection (IO redirection).
IO redirection can only be enabled when QoS is disabled.

Currently the driver doesn't take into consideraion of possible sg chaining
when it walks through the sg list. This is fixed by using the sg_next()
which automatically handles the chaining case. Obosolete code is removed
as a result of this change.
