Extend bat_xl handling to do err_xl handling, so that
keyboards using 0x7f scancode for regular keys can work.

Rearrange attribute code to use generic show and set handlers
instead of replicating them for every attribute; switch to
using attribute_group instead of creating all attributes
manually. All this saves about 4K.

Use MAP_PRIVATE when calling mmap to get memory for the code region.
The flat loader was using MAP_SHARED, but underlying changes to the
MMUless mmap means this is now wrong.

We weren't explicitly setting the page table bits we desired
in user_prot in the protection table, which resulted in the
user mappings for v6 CPUs being marked global.
