The set_memory_* and set_pages_* family of API's currently requires the
callers to do a global tlb flush after the function call; forgetting this is
a very nasty deathtrap. This patch moves the global tlb flush into
each of the callers

For enhancing the 32 bit EBP based backtracer, I need the capability
for the backtracer to tell it's customer that an entry is either
reliable or unreliable, and the backtrace printing code then needs to
print the unreliable ones slightly different.

This patch adds the basic capability, the next patch will add a user
of this capability.

This patch renames the 4 symbols iommu_hole_init(), iommu_aperture,
iommu_aperture_allowed, iommu_aperture_disabled. All these symbols are only
used for the GART implementation of IOMMUs.

blk_rq_map_sg doesn't initialize sg->dma_address/length to zero
anymore. Some low level drivers reuse sg lists without initializing so
IOMMUs might get non-zero dma_address/length. If map_sg fails, we need
pass the number of the mapped entries to gart_unmap_sg.

map_sg could copy the last sg element to another position (if merging
some elements). It breaks sg chaining. This copies only
dma_address/length instead of the whole sg element.

For K8 system: 4G RAM with memory hole remapping enabled, or more than 4G
RAM installed.  when using kexec to load second kernel.  In the second
kernel, when mem is allocated for GART, it will do the memset for clear, it
will cause restart, because some device still used that for dma.  solution
will be:

in second kernel: disable that at first before we try to allocate mem for
it.  or in the first kernel: do disable that before shutdown.
Andi/Eric/Alan prefer to second one for clean shutdown in first kernel.
Andi also point out need to consider to AGP enable but mem less 4G case
too.

I'm using a custom BIOS to configure the northbridge GART at address
0x80000000, size 2G.  Linux complains:

"Aperture from northbridge cpu 0 beyond 4GB. Ignoring."

This patch moves the die notifier handling to common code.  Previous
various architectures had exactly the same code for it.  Note that the new
code is compiled unconditionally, this should be understood as an appel to
the other architecture maintainer to implement support for it aswell (aka
sprinkling a notify_die or two in the proper place)

arm had a notifiy_die that did something totally different, I renamed it to
arm_notify_die as part of the patch and made it static to the file it's
declared and used at.  avr32 used to pass slightly less information through
this interface and I brought it into line with the other architectures.

This patches fixes the silent data corruption problems being seen using the
GART iommu where 4kB of data where incorrect (seen mostly on Nvidia CK804
systems).  This fix, to mark the memory regin the GART PTEs reside on as
uncacheable, also brings the code in line with the AGP specification.

This patch creates a new interface for IOMMUs by adding a centralized
location for IOMMU allocation (for translation tables/apertures) and
IOMMU initialization.  In creating these, code was moved around for
abstraction, uniformity, and consiceness.

Take note of the move of the iommu_setup bootarg parsing code to
__setup.  This is enabled by moving back the location of the aperture
allocation/detection to mem init (which while ugly, was already the
location of the swiotlb_init).

While a slight departure from the previous patch, I belive this provides
the true intention of the previous versions of the patch which changed
this code.  It also makes the addition of the upcoming calgary code much
cleaner than previous patches.

swiotlb relies on the gart specific iommu_aperture variable to know if
we discovered a hardware IOMMU before swiotlb initialization.  Introduce
iommu_detected to do the same thing, but in a HW IOMMU neutral manner,
in preparation for adding the Calgary HW IOMMU.

A trivial change to have gart_unmap_sg call gart_unmap_single directly,
instead of bouncing through the dma_unmap_single wrapper in
dma-mapping.h.

This change required moving the gart_unmap_single above gart_unmap_sg,
and under gart_map_single (which seems a more logical place that its
current location IMHO).

We do this by removing a micro-optimization that tries to avoid grabbing
the iommu_bitmap_lock spinlock and using a bus-locked operation.

This still races with other simultaneous alloc_iommu or free_iommu(size >
1) which both use bus-unlocked operations.

The end result of this race is eventually ending up with an
iommu_gart_bitmap that has bits errornously set all over, making large
contiguous iommu space allocations fail with 'PCI-DMA: Out of IOMMU space'.

arch/x86_64/kernel/aperture.c: The search for the AGP bridge has been
extended to search for all the 256 buses instead of the first 32. This
is required since on a some systems, the bridge may be located on a bus
much farther than the first 32. By searching all 256 buses, we guarantee
that the search succeeds on such systems.

arch/x86_64/kernel/pci-gart.c: The search for the Northbridge is not
limited to just bus 0 anymore. This is required because on certain
systems, we may not find one on bus 0.

In previous versions of pci-gart.c, no_iommu was used to determine if IOMMU was
disabled in the GART DMA mapping functions.  This changed in 2.6.16 and now
gart_xxx() functions are only called if gart is enabled.  Therefore, uses of
no_iommu in the GART code are no longer necessary and can be removed.

Also, it removes double deceleration of no_iommu and force_iommu in pci.h and
proto.h, by removing the deceleration in pci.h.

Lastly, end_pfn off by one error.

Tested (along with patch 1/2) on dual opteron with gart enabled, iommu=soft,
and iommu=off.
