Change the 5th argument of snd_mpu401_uart_new() to bit flags
instead of a boolean.  The argument takes bits that consist of
MPU401_INFO_XXX flags.
The callers that used the value 1 there are replaced with
MPU401_INFO_INTEGRATED.

Replace all occurences of 0xff..  in calls to function pci_set_dma_mask()

This patch fixes the suspend/resume issue I'm having with ESS-Solo1
soundcard. Without this patch I might get after resume message
that kernel is disabling the IRQ5 (soundcard). If there was something
playing it wont continue after resume without this patch.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.

Let it rip!
