Add a reg_mutex to prevent unregistering a subchannel before it has been registered. Since 2.6.17, we've seen oopses in kslowcrw when a device is found to be not operational during sense id when doing initial device recognition; it is not clear yet why that particular problem was not (yet) observed with earlier kernels... This patch introduces a struct subchannel_id containing the subchannel number (formerly referred to as "irq") and switches code formerly relying on the subchannel number over to it. While we're touching inline assemblies anyway, make sure they have correct memory constraints. Initial git repository build. I'm not bothering with the full history, even though we have it. We can create a separate "historical" git archive of that later if we want to, and in the meantime it's about 3.2GB when imported into git - space that would just make the early git days unnecessarily complicated, when we don't have a lot of good infrastructure for it. Let it rip!
