switch early init to init_early and introduce soc map_io

with this Patch we will not do any more early device setup during the map io

tks to Russell to point the new call back

Since we're now using addruart to establish the debug mapping, we can
remove the io_pg_offst and phys_io members of struct machine_desc.

The various declarations were removed using the following script:

  grep -rl MACHINE_START arch/arm | xargs \
  sed -i '/MACHINE_START/,/MACHINE_END/ { /\.\(phys_io\|io_pg_offst\)/d }'

CONFIG_MACH_ECO920 is enabled in at91rm9200dk_defconfig.  The name is
wrong, but this is better than adding another defconfig or don't get
compile coverage at all.
