Coverity-scan recently found a possible NULL pointer dereference in fc_block_scsi_eh() as starget_to_rport() either returns the rport for the startget or NULL. While it is rather unlikely to have fc_block_scsi_eh() called without an rport associated it's a good idea to catch potential misuses of the API gracefully. During failover there is a small race window between fc_remote_port_add() and fc_timeout_deleted_rport(); the latter drops the lock after setting the port to NOTPRESENT, so if fc_remote_port_add() is called right at that time it will fail to detect the existing rport and happily adding a new structure, causing rports to get registered twice. When an rport is found in the bindings array there is no guarantee that it had been a target port, so we need to call fc_remote_port_rolechg() here to ensure the scsi_target_id is set correctly. Otherwise the port will never be scanned. Scsi_cmnd is an unsuitable argument for eh_device_reset_handler(), eh_target_reset_handler(), and eh_host_reset_handler() which do not have the scope of one single SCSI command. These callbacks tend to use fc_block_scsi_eh() requiring scsi_cmnd. In order to start decoupling above eh callbacks from scsi_cmnd, introduce a new variant of the function called fc_block_rport() taking an fc_rport as argument. Refactor the old fc_block_scsi_eh() to simply delegate to fc_block_rport(). This patch allows scsi drivers that expose virturalized fibre channel devices but that do not expose rports to successfully rescan the scsi bus via echo "- - -" > /sys/class/scsi_host/hostX/scan. Drivers can create a pseudo rport and indicate FC_PORT_ROLE_FCP_DUMMY_INITIATOR as the rport's role in fc_rport_identifiers. This insures that a valid scsi_target_id is assigned to the newly created rport and it can meet the requirements of fc_user_scan_tgt calling scsi_scan_target. Instead define the timeout behavior purely based on the host_template eh_timed_out method and wire up the existing transport implementations in the host templates. This also clears up the confusion that the transport template method overrides the host template one, so some drivers have to re-override the transport template one. Export fc_bsg_jobdone so drivers can use it directly instead of doing the round-trip via struct fc_bsg_job::job_done() and use it in the LLDDs. That way we can also unify the interfaces of fc_bsg_jobdone and bsg_job_done. It is required to hold the queue lock when calling blk_run_queue_async() to avoid that a race between blk_run_queue_async() and blk_cleanup_queue() is triggered. Additionally, remove the get_device() and put_device() calls from fc_bsg_goose_queue. It is namely the responsibility of the caller of fc_bsg_goose_queue() to ensure that the bsg queue does not disappear while fc_bsg_goose_queue() is in progress. On larger installations it is useful to disable automatic LUN scanning, and only add the required LUNs via udev rules. This can speed up bootup dramatically. This patch introduces a new scan module parameter value 'manual', which works like 'none', but can be overridden by setting the 'rescan' value from scsi_scan_target to 'SCSI_SCAN_MANUAL'. For the workqueue creation interfaces that do not expect format strings, make sure they cannot accidently be parsed that way. Additionally, clean up calls made with a single parameter that would be handled as a format string. Many callers are passing potentially dynamic string content, so use "%s" in those cases to avoid any potential accidents.
