task #28557685 commit 7c990728b99ed6fbe9c75fc202fce1172d9916da upstream. During an online resize an array of s_flex_groups structures gets replaced so it can get enlarged. If there is a concurrent access to the array and this memory has been reused then this can lead to an invalid memory access. The s_flex_group array has been converted into an array of pointers rather than an array of structures. This is to ensure that the information contained in the structures cannot get out of sync during a resize due to an accessor updating the value in the old structure after it has been copied but before the array pointer is updated. Since the structures them- selves are no longer copied but only the pointers to them this case is mitigated. That means that even kernels / tools that don't understand the feature may modify the filesystem. This works because for kernels not understanding indexed dir format, internal htree nodes appear just as empty directory entries. Index dir aware kernels then check the htree structure is still consistent before using the data. This all worked reasonably well until metadata checksums were introduced. The problem is that these effectively made DIR_INDEX only ro-compatible because internal htree nodes store checksums in a different place than normal directory blocks. Thus any modification ignorant to DIR_INDEX (or just clearing EXT4_INDEX_FL from the inode) will effectively cause checksum mismatch and trigger kernel errors. So we have to be more careful when dealing with indexed directories on filesystems with checksumming enabled. 1) We just disallow loading any directory inodes with EXT4_INDEX_FL when DIR_INDEX is not enabled. This is harsh but it should be very rare (it means someone disabled DIR_INDEX on existing filesystem and didn't run e2fsck), e2fsck can fix the problem, and we don't want to answer the difficult question: "Should we rather corrupt the directory more or should we ignore that DIR_INDEX feature is not set?" 2) When we find out htree structure is corrupted (but the filesystem and the directory should in support htrees), we continue just ignoring htree information for reading but we refuse to add new entries to the directory to avoid corrupting it more. A recent commit, 9803387c55f7 ("ext4: validate the debug_want_extra_isize mount option at parse time"), moved mount-time checks around. One of those changes moved the inode size check before the blocksize variable was set to the blocksize of the file system. After 9803387c55f7 was set to the minimum allowable blocksize, which in practice on most systems would be 1024 bytes. task #28557685 [ Upstream commit 547c556f4db7c09447ecf5f833ab6aaae0c5ab58 ] ext4_writepages() on an encrypted file has to encrypt the data, but it can't modify the pagecache pages in-place, so it encrypts the data into bounce pages and writes those instead. All bounce pages are allocated from a mempool using GFP_NOFS. This is not correct use of a mempool, and it can deadlock. This is because GFP_NOFS includes __GFP_DIRECT_RECLAIM, which enables the "never fail" mode for mempool_alloc() where a failed allocation will fall back to waiting for one of the preallocated elements in the pool. But since this mode is used for all a bio's pages and not just the first, it can deadlock waiting for pages already in the bio to be freed. This deadlock can be reproduced by patching mempool_alloc() to pretend that pool->alloc() always fails (so that it always falls back to the preallocations), and then creating an encrypted file of size > 128 KiB. Fix it by only using GFP_NOFS for the first page in the bio. For subsequent pages just use GFP_NOWAIT, and if any of those fail, just submit the bio and start a new one. This will need to be fixed in f2fs too, but that's less straightforward. This can lead to directory iteration code trying to access address beyond end of current buffer head leading to oops. task #28557808 [ Upstream commit 3a8ecc935efabdad106b5e06d07b150c394b4465 ] Commit 7fd8930f "nvme: add a common helper to read Identify Controller data" has re-introduced an issue that we have attempted to work around in the past, in commit a310acd7 ("NVMe: use split lo_hi_{read,write}q"). The problem is that some PCIe NVMe controllers do not implement 64-bit outbound accesses correctly, which is why the commit above switched to using lo_hi_[read|write]q for all 64-bit BAR accesses occuring in the code. In the mean time, the NVMe subsystem has been refactored, and now calls into the PCIe support layer for NVMe via a .reg_read64() method, which fails to use lo_hi_readq(), and thus reintroduces the problem that the workaround above aimed to address. Given that, at the moment, .reg_read64() is only used to read the capability register [which is known to tolerate split reads], let's switch .reg_read64() to lo_hi_readq() as well. This fixes a boot issue on some ARM boxes with NVMe behind a Synopsys DesignWare PCIe host controller. Two different variants of this behavior have been observed: SM22XX controllers round up the read size to a multiple of 512 bytes, and Phison E12 unconditionally reads the maximum discard size allowed by the spec (256 segments or 4kB). Make nvme_setup_discard unconditionally allocate the maximum DSM buffer so the driver DMA maps a memory range that will always succeed. When adding devices that don't have a scsi_dh on a BIO based multipath, I was able to consistently hit the warning below and lock-up the system. The problem is that __map_bio reads the flag before it potentially being modified by choose_pgpath, and ends up using the older value. The WARN_ON below is not trivially linked to the issue. It goes like this: The activate_path delayed_work is not initialized for non-scsi_dh devices, but we always set MPATHF_QUEUE_IO, asking for initialization. That is fine, since MPATHF_QUEUE_IO would be cleared in choose_pgpath. Nevertheless, only for BIO-based mpath, we cache the flag before calling choose_pgpath, and use the older version when deciding if we should initialize the path. There are variants for tracking metadata blocks and data blocks. Transactionality is implemented by never touching blocks from the previous transaction, so we can rollback in the event of a crash. When allocating a new block we need to ensure the block is free (has reference count of 0) in both the current and previous transaction. Prior to this fix we were doing this by searching for a free block in the previous transaction, and relying on a 'begin' counter to track where the last allocation in the current transaction was. This 'begin' field was not being updated in all code paths (eg, increment of a data block reference count due to breaking sharing of a neighbour block in the same btree leaf). This fix keeps the 'begin' field, but now it's just a hint to speed up the search. Instead the current transaction is searched for a free block, and then the old transaction is double checked to ensure it's free. Much simpler. This fixes reports of sm_disk_new_block()'s BUG_ON() triggering when DM thin-provisioning's snapshots are heavily used. Since nobody else is going to restart our hw_queue for us, the blk_mq_start_stopped_hw_queues() is in virtblk_done() is not sufficient necessarily sufficient to ensure that the queue will get started again. In case of global resource outage (-ENOMEM because mapping failure, because of swiotlb full) our virtqueue may be empty and we can get stuck with a stopped hw_queue. Let us not stop the queue on arbitrary errors, but only on -EONSPC which indicates a full virtqueue, where the hw_queue is guaranteed to get started by virtblk_done() before when it makes sense to carry on submitting requests. Let us also remove a stale comment. The bfqq, which is not equal to NULL in bfq_idle_slice_timer, may be freed after passing to bfq_idle_slice_timer_body. So we will access the freed memory. In addition, considering the bfqq may be in race, we should firstly check whether bfqq is in service before doing something on it in bfq_idle_slice_timer_body func. If the bfqq in race is not in service, it means the bfqq has been expired through __bfq_bfqq_expire func, and wait_request flags has been cleared in __bfq_bfqd_reset_in_service func. So we do not need to re-clear the wait_request of bfqq which is not in service. Stacked drivers do not call blk_queue_max_hw_sectors(). Instead they set limits of their devices by blk_set_stacking_limits() + disk_stack_limits(). Field bio->io_pages stays zero until user set max_sectors_kb via sysfs. This patch updates io_pages after merging limits in disk_stack_limits(). Commit c6d6e9b0f6b4 ("dm: do not allow readahead to limit IO size") fixed the same problem for device-mapper devices, this one fixes MD RAIDs. task #28557799 [ Upstream commit 14afc59361976c0ba39e3a9589c3eaa43ebc7e1d ] The bfq_find_set_group() function takes as input a blkcg (which represents a cgroup) and retrieves the corresponding bfq_group, then it updates the bfq internal group hierarchy (see comments inside the function for why this is needed) and finally it returns the bfq_group. In the hierarchy update cycle, the pointer holding the correct bfq_group that has to be returned is mistakenly used to traverse the hierarchy bottom to top, meaning that in each iteration it gets overwritten with the parent of the current group. Since the update cycle stops at root's children (depth = 2), the overwrite becomes a problem only if the blkcg describes a cgroup at a hierarchy level deeper than that (depth > 2). In this case the root's child that happens to be also an ancestor of the correct bfq_group is returned. The main consequence is that processes contained in a cgroup at depth greater than 2 are wrongly placed in the group described above by BFQ. This commits fixes this problem by using a different bfq_group pointer in the update cycle in order to avoid the overwrite of the variable holding the original group reference. However, there are architectures that can run with 64k pages (for example arm64) and on these architectures, it may be possible to create block devices with 64k block size. For exmaple (run this on an architecture with 64k pages): Mount will fail with this error because it tries to read the superblock using 2-sector access: device-mapper: writecache: I/O is not aligned, sector 2, size 1024, block size 65536 EXT4-fs (dm-0): unable to read superblock This patch changes the logical block size from unsigned short to unsigned int to avoid the overflow. fix #27212891 Fix the bug when calculating the physical block number of the first block in the split extent. This bug will cause xfstests shared/298 failure on ext4 with bigalloc enabled occasionally. Ext4 error messages indicate that previously freed blocks are being freed again, and the following fsck will fail due to the inconsistency of block bitmap and bg descriptor. fix #28528017 In case of virtio-blk device, checking /sys/block/<device>/queue/io_poll will show 1 and user can't disable it. Actually virtio-blk doesn't support poll yet, so it will confuse end user. The root cause is mq initialization will default set bit QUEUE_FLAG_POLL. This fix takes ideas from the following upstream commits: 6544d229bf43 ("block: enable polling by default if a poll map is initalized") 6e0de61107f0 ("blk-mq: remove QUEUE_FLAG_POLL from default MQ flags") Since we don't want to get HCTX_TYPE_POLL related logic involved, so just check mq_ops->poll and then set QUEUE_FLAG_POLL. The submission part can be significant if backing storage is contended or when cgroup throttling (io.latency) is in effect - a lot of time is spent in submit_bio(). In that case, we underreport memory pressure. Annotate submit_bio() to account submission time as memory stall when the bio is reading userspace workingset pages. task #28327019 commit 36b238d5717279163859fb6ba0f4360abcafab83 upstream When switching tasks running on a CPU, the psi state of a cgroup containing both of these tasks does not change. Right now, we don't exploit that, and can perform many unnecessary state changes in nested hierarchies, especially when most activity comes from one leaf cgroup. This patch implements an optimization where we only update cgroups whose state actually changes during a task switch. These are all cgroups that contain one task but not the other, up to the first shared ancestor. When both tasks are in the same group, we don't need to update anything at all. We can identify the first shared ancestor by walking the groups of the incoming task until we see TSK_ONCPU set on the local CPU; that's the first group that also contains the outgoing task. The new psi_task_switch() is similar to psi_task_change(). To allow code reuse, move the task flag maintenance code into a new function and the poll/avg worker wakeups into the shared psi_group_change(). The period, stored in R8, is not 0, but we are dividing by the lower 32 bits only, which are all 0 in this instance. We could switch to a 64-bit division, but the period shouldn't be that big in the first place. It's the time between the last update and the next scheduled one, and so should always be around 2s and comfortably fit into 32 bits. The bug is in the initialization of new cgroups: we schedule the first sampling event in a cgroup as an offset of sched_clock(), but fail to initialize the last_update timestamp, and it defaults to 0. That results in a bogusly large sampling period the first time we run the sampling code, and consequently we underreport pressure for the first 2s of a cgroup's life. But worse, if sched_clock() is sufficiently advanced on the system, and the user gets unlucky, the period's lower 32 bits can all be 0 and the sampling division will crash. Fix this by initializing the last update timestamp to the creation time of the cgroup, thus correctly marking the start of the first pressure sampling period in a new cgroup.
