NOP transceiver is used by all the usb transceiver which are mostly
autonomous and doesn't require any programming or which are built
into the usb ip itself.NOP transceiver only allocates the memory
for struct xceiv and calls otg_set_transceiver() so function call
to otg_get_transceiver() will return a valid transceiver.

NOP transceiver device should be registered by calling
usb_nop_xceiv_register() from platform files.

Call usb_gadget_vbus_connect() and ...disconnect() from a
workqueue rather than from an irq handler, allowing msleep()
calls in vbus_session.  Update kerneldoc to match.

For the header files in include/linux/usb, add missing multiple
inclusion protection and standardize what's already there.  The
apparent standards:

  * macro name of __LINUX_USB_headerfile_H
  * inclusion protection placed after leading comment block
  * macro name added as a comment on the final #endif
  * any obvious trivial whitespace cleanup associated with the above

When a USB device is put into suspend mode, the current drawn from VBUS
has to be less than 500 uA. Some transceivers need to be put into a
special power-saving mode to accomplish this, and won't have a separate
OTG driver handling that.

This adds a suspend method to the "otg_transceiver" struct -- misnamed,
it's not only for OTG -- and calls it from the OMAP UDC driver.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.

Let it rip!
