Parameters delay_on and delay_off of led_trigger_blink_oneshot()
are pointers, to enable blink interval adjustment by LED class drivers
of the controllers that implement hardware blinking.

Move ide_blink_delay variable to ledtrig_ide_activity() in order to
prevent the situation when adjustment committed by one LED class
driver influences blink interval of the software fallback blink feature,
that is applied to the drivers that don't implement blink_set op.

The Kconfig for this driver is currently:

config LEDS_TRIGGER_IDE_DISK
        bool "LED IDE Disk Trigger"

...meaning that it currently is not being built as a module by anyone.
Lets remove the modular code that is essentially orphaned, so that
when reading the driver there is no doubt it is builtin-only.

Since module_init translates to device_initcall in the non-modular
case, the init ordering remains unchanged with this commit.

We also delete the MODULE_LICENSE tag etc. since all that information
is already contained at the top of the file in the comments.

For better driver management, new subdirectory, 'trigger' is created.
All LED trigger drivers are moved into this directory.

Internal header, 'leds.h' is included in each LED trigger drivers.
Fix the location of header file, "leds.h" -> "../leds.h" in driver files.
One exception is here, 'ledtrig-timer.c'.
There is no need to include 'leds.h'. so '#include "leds.h"' line was removed.

Convert ledtrig-ide-disk code to use the generic API for one-shot LED
blinking.

This patch changes slightly the behaviour of the trigger, as while the
original version kept the LED on under heavy activity, the new one keeps
a constant on-off blink at 1 / (2 * BLINK_DELAY) Hz.

This patch allows drivers to override the default maximum brightness value
of 255.  We take care to preserve backwards-compatibility as much as
possible, so that user-space ABI doesn't change for existing drivers.
LED trigger code has also been updated to use the per-LED maximum.

Break the lines which were more than 80 characters into more
lines; replace SPACEs with TABs; correct ident at switch-case;
change character encoding from ISO-8859-2 to UTF-8.

The order of the functions in led-triggers.c changed in order
the similar functions can still be together under titles
"Used by LED Class", "LED Trigger Interface" and "Simple
LED Tigger Interface" as was grouped before when exported
with EXPORT_SYMBOL.
