EXCEPTION_PROLOG_PSERIES() only has two users, STD_EXCEPTION_PSERIES()
and STD_EXCEPTION_HV() both of which "call" SET_SCRATCH0(), so just
move SET_SCRATCH0() into EXCEPTION_PROLOG_PSERIES().
