Even though we don't build a Binaries directory anymore lots of
developers will have this on their machine. To avoid any accidentally
adding of Binarise to our history putting it in the ignore list. Can
remove this some time down the road when it's unlikely anyone has
Binaries directories remaining on their machine.

We accidentally did a call to GetDocument passing it an additional
document's ID, which was causing us to immediately filter out the
events. We also weren't calling the right helper to provide the new
TextLoader when we needed to.

This is especially helpful when the client wants to perform chain of edits such that they don't need to track which nodes are in the original tree and which are the intermediate new nodes generated during their prior Replace/Insert invocations on the editor.
