[PATCH] libata: kill NULL qc handling from ->eng_timeout callbacks

->eng_timeout cannot be invoked with NULL qc anymore.  Add an
assertion in ata_scsi_error() and kill NULL qc handling from all
->eng_timeout callbacks.
