Remove the file name from the comment at top of many files. In most
cases the file name was wrong anyway, so it's rather pointless.

Also unify the IBM copyright statement. We did have a lot of sightly
different statements and wanted to change them one after another
whenever a file gets touched. However that never happened. Instead
people start to take the old/"wrong" statements to use as a template
for new files.
So unify all of them in one go.

Currently qemu/kvm on s390 uses a guest mapping that does not
allow the guest backing page table to be write-protected to
support older systems. On those older systems a host write
protection fault will be delivered to the guest.

Newer systems allow to write-protect the guest backing memory
and let the fault be delivered to the host, thus allowing COW.

Use a capability bit to tell qemu if that is possible.

Add missing storage key initialization when memory comes online.
Didn't hurt until now, since the kernel always uses access key 0.

However for debugging purposes we sometimes use a different key
and it might be good if everything is properly initialized...

Obtain tty_struct only once in ISR and pass it down to
raw3215_next_io. Other than that, we just use the tty with raised
reference. And set it properly in open and close.

This patch is used to fix a memory leak issue in s390/char/vmur.c.
A character device instance is allocated by cdev_alloc, the cdev_del
will not free that space if cdev_init is applied before.

There are two copies of allocations of device information. One of them
is totally broken. See:
raw->cdev = cdev;
raw->inbuf = (char *) raw + sizeof(struct raw3215_info);
memset(raw, 0, sizeof(struct raw3215_info));

It suggests that this path was never executed. The code uses both
raw->cdev and raw->inbuf all over. And it is NULL due to the memset
here, so it would panic immediately. I believe nobody used that driver
without being a system console.

Either way, let us fix it by moving the allocations (and
initializations) to a single place. This will save us some double
initializations later too.

And while at it, initialize the timer properly -- once, at the
allocation.

The external interrupt handlers have a parameter called ext_int_code.
Besides the name this paramter does not only contain the ext_int_code
but in addition also the "cpu address" (POP) which caused the external
interrupt.
To make the code a bit more obvious pass a struct instead so the called
function can easily distinguish between external interrupt code and
cpu address. The cpu address field however is named "subcode" since
some external interrupt sources do not pass a cpu address but a
different parameter (or none at all).

Define struct pcpu and merge some of the NR_CPUS arrays into it, including
__cpu_logical_map, current_set and smp_cpu_state. Split smp related
functions to those operating on physical cpus and the functions operating
on a logical cpu number. Make the functions for physical cpus use a
pointer to a struct pcpu. This hides the knowledge about cpu addresses in
smp.c, entry[64].S and swsusp_asm64.S, thus remove the sigp.h header.

The PSW restart mechanism is used to start secondary cpus, calling a
function on an online cpu, calling a function on the ipl cpu, and for
the nmi signal. Replace the different assembler functions with a
single function restart_int_handler. The new entry point calls a function
whose pointer is stored in the lowcore of the target cpu and it can wait
for the source cpu to stop. This covers all existing use cases.

Overall the code is now simpler and there are ~380 lines less code.

Checking if tty->index is in bounds is not needed. The tty has the
index set in the initial open. This is done in get_tty_driver. And it
can be only in interval <0,driver->num).

So remove the tests which check exactly this interval. Some are
left untouched as they check against the current backing device count.
(Leaving apart that the check is racy in most of the cases.)

All num, magic and owner are set by alloc_tty_driver. No need to
re-set them on each allocation site.

pti driver sets something different to what it passes to
alloc_tty_driver. It is not a bug, since we don't use the lines
parameter in any way. Anyway this is fixed, and now we do the right
thing.

The 3215 driver calls tty_wakeup from irq context while holding the
device spinlock. If printk is called by any function on the callchain
starting from tty_wakeup the system deadlocks on the device spinlock.
Using a tasklet to call tty_wakup solves the problem.

module_param(bool) used to counter-intuitively take an int.  In
fddd5201 (mid-2009) we allowed bool or int/unsigned int using a messy
trick.

It's time to remove the int/unsigned int option.  For this version
it'll simply give a warning, but it'll break next kernel version.

This moves the 'cpu sysdev_class' over to a regular 'cpu' subsystem
and converts the devices to regular devices. The sysdev drivers are
implemented as subsystem interfaces now.

After all sysdev classes are ported to regular driver core entities, the
sysdev implementation will be entirely removed from the kernel.

Userspace relies on events and generic sysfs subsystem infrastructure
from sysdev devices, which are made available with this conversion.

to PSW_MASK_BASE, get rid of psw_user32_bits, remove unused function
enabled_wait(), introduce PSW_MASK_USER, and drop PSW_MASK_MERGE macros.
Change psw_kernel_bits / psw_user_bits to contain only the bits that

Current IRQ statistics support does not show detail counts for I/O
interrupts which are processed internally only. The result is a
summation count which is way off such as this one:
