readahead: Update comments

 - Refer to folios where appropriate, not pages (Matthew Wilcox)
 - Eliminate references to the internal PG_readhead
 - Use "readahead" consistently - not "read-ahead" or "read ahead"
   (mostly Neil Brown)
 - Clarify some sections that, on reflection, weren't very clear (Neil

 - Minor punctuation/spelling fixes (Neil Brown)
